Part 1 Basic information in form ”Form to send to your unemployment insurance fund” as well as Part 1 Basic information in form ”Form to send to the Swedish Public Employment Service”.
When you have filled out both of the forms you should send one to your unemployment insurance fund and one to the Swedish Public Employment Service (Arbetsförmedlingen, EU-intyg) by mail. The unemployment insurance fund and the Swedish Public Employment Service will fill out Part 2.
Once the forms have been returned to you, check that they are complete and sign both of them in Part 3.
Then send both the forms for applying for a U2 certificate to the IAF.
In order for the IAF to be able to process your application before your departure we recommend you to hand in your fully completed application to the IAF no later than five working days before your date of departure.
When one of the forms has been returned to you, from either your unemployment insurance fund or the Swedish Public Employment Service, you can send that form to the IAF as soon as you checked the information and signed part 3. If you do this be sure to inform us that you will send the other form to the IAF as soon as you received it.
